New Student Applications are being accepted NOW! Monday-Thursday for the 2012-2013 School Year from 9:00 AM to 2:00 PM.

All forms are available for download on the right side of this page.  Please refer to the New Student Application Check list (PDF document) toyour right, as a guide on how to complete the enrollment packet correctly.  Applications will be accepted on a first come, first serve basis.  Incomplete enrollment packets will not be accepted. All packets must include every form (listed on the checklist).  PLEASE PROVIDE ALL DOCUMENTS LISTED BELOW AT ONE TIME. Thank you for your compliance.

Registration forms and documentation must be submitted in person to a school official in the Main Office located at:

Please bring the following documentation with you:
 

• Completed and signed New Student Application (2 pages)*
• IF RESIDENCY IS NOT IN YOUR NAME; Signed and notarizedAtlanta Public Schools Affidavit of Residency (2 pages)* by Parent/Guardian and residency owner

OR
 

• IF OWN, RENT OR LEASE RESIDENCY; signed and notarizedGeneral Enrollment Affidavit*
• Signed Healthy Student Form*
• Copy of a mortgage statement or lease agreement with your Atlanta Public School zoned address
• Copy of electric bill with your Atlanta Public School zoned address (dated within 30 days of applying)
• Copy of your driver’s license or other valid photo ID with your Atlanta Public School zoned address
• Student’s official birth certificate (we will copy and return original)
• Copy of the student’s social security card with the social security number OR Form IS-2, Statement of Objection to the Use of Social Security Number for Student Identification*
• Georgia Department of Human Resources Form 3231- Certificate of Immunization showing that the child has adequate protection against varicella (chicken pox), Hepatitis B, diphtheria, pertussis (whooping cough), tetanus (lockjaw), polio, measles, rubella (German measles) and mumps (dated within 30 days of applying). The Certificate of Immunization (Form 3231) must be signed by a physician licensed under the laws of Georgia OR by a qualified employee of a county health department. A child who cannot be immunized because of a medical disability can be given a special certificate by a licensed physician or a county health department. If a child is not immunized for religious reasons, please use the Objection of Immunization Affidavit. An expired immunization form will not be accepted. MUST BE READABLE.
• Form 3300 - Certificate of Ear, Eye and Dental(dated within 30 days of applying, and please ensure there is a signature by each section)

• Rising 1st-8th grade students, copy of 2011-12 SY Report Card (bring most current)
• Rising 2nd-8th grade students, copy of 2011-12 CRCT Test Results (bring most current)
• Rising 1st-8th grade students, Discipline Report (bring most current)
